Anyone living within our practice boundary (YO11, YO12 and YO13) is welcome to register with the practice. You can register by completing a form at reception during surgery hours.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Patients are registered with the practice and not with a specific doctor although when calling for an appointment you will be given an option to see any doctor working at the practice.
Newly registered patients will be asked to complete a health questionnaire and a registration form and they will then be able to access the services at the surgery.
